Is there any way to stop it coming back or have the seals gone?

Water will be gettng in somewhere but we have had alot of rain. To re seal them again will be a pita as you have to take your bumper off to get the headlights out :(

Are the rubbers on the back of your bulbs on properly??

One way to stop the condensation, when bulbs are out, try and fit in the lens area one of those small packets of silicon you get in deliveries of some things, i used to get them in camera lenses/bodies, obviously it would need securing and in a place that is possibly hidden.

Its a thought, never done it myself, but looking at my lenses now, i might have to...

Yes Rich, it's silica gel. I put a bag in each of mine when I cleaned them and refitted my chrome inserts. The silica bags sit behind the insert so there's no chance of them being seen

The seal/s are just Sikaflex/Tigerseal silicone if you remove your headlights as others have suggested you then need to heat up the silicone remove the lense and then apply alittle more Sikaflex between the two surfaces then re-attach the lense.
