Gearbox question

Well I think my gearbox has died (properly this time)

 

I have done lots of searches and people talk about the car making more noise when changing up from 3rd but not going any fast.

 

Mine simply wont change above 2nd (as if I have left it in 2). So I can do 60 MPH but it sits at just over 4000 revs.

 

Is it time to book it in for a gearbox?

    High clutch pack failure it sounds like... Change fluid, you might get lucky... But it sounds deaadddd to me!
