Front strut top wear

Has anyone here replaced a worn strut top? I have what I think is a worn near side one. The main shock nut appears not to be in dead-centre like it does on the offside, in addition I get a very faint squelching sound from that area at low speed over speed humps.

 

Just wondering if anyone else has had this problem before? I've had it on a car I owned years ago and the symptoms are similar.
