dosnt look to bad .. it may be just a weak pressure plate ..

there is oil'y deposits on both fly and p-plate

is there a sign of an oil leak ?

There has definately been slipping going on, evidence is clear to see on both the flywheel and the clutch cover, the set up does looks quite stock to be fair, hard to be certain but the contact area on the flywheel should be raised but it looks quite worn, this will reduce the unloaded grip and could be part of the problem as could be a wrongly adjusted clutch pedal.

They are probably very hard to come by as most people sell them as part of the auto to manual conversion kit.
